This is to bring to your kind notice that I had applied for a telephone connection three months back and has deposited the requisite fee etc. A photocopy of the receipt is attached herewith. My mother keeps unwell and in the event of any emergency, I would find myself in a great difficulty. There is no PCO booth also anywhere nearby.
I had already met you personally in this connection twice and you had told me that the telephone connection has been sanctioned and the telephone would be installed soon. But I have been waiting in vain for it now for a month and more. Considering the emergency of the situation as noted above, I would request you to please have the telephone installed at the earliest.
I will be highly obliged.
