Re: Request for issuance of fresh Ration Cards in lieu of old ones,

Dear Sir,

This is to inform you that I am the holder of four Ration Cards in the names of myself/ my wife and two sons. These Ration cards which were issued by your Office on____________ (Date) have become spoiled due to constant use writings on the same are not at all readable for which the Ration Shop owner is finding it extremely difficult to give ration against these Ration Cards.

These Ration Cards, therefore, need replacement with the issuance of fresh ones. The names and addresses with other details to be mentioned in the replaced Ration Cards are as follows:

While enclosing the old Ration Cards, I would request you to arrange to issue new ones in replacement of the same.
