Dear Sir,

This is to inform you that the name of my wife, ____________ (Name) was incorporated in my Policy No. ____________ as nominee. I regret to inform you that she expired on ____________ (Date) as per the death certificate dated _________ enclosed. Since I have again married after the -death of my wife, I have felt the necessity of getting the name of my newly married wife incorporated in my above Policy as nominee.

In this connation, I am enclosing consent letter dated ____________ of my wife ____________ (name of my wife) desiring to become nominee in my Policy.

I would now request you to arrange to incorporate her name in my above Policy as nominee after deleting the name of my deceased wife from the Policy and inform me having done so.
